In Bangladesh, almost everyone selling websites calls their work "development," so the word has stopped meaning much. The line that actually matters is this: is the site being built to do what your business needs, or is a ready-made theme being bent to roughly fit and called custom?

A theme job is fast and cheap on day one, but the moment you want something the theme wasn't designed for, a specific booking flow, a particular product filter, a back-end the rest of your tools can talk to, you hit a wall, and the only fix is paying to redo it properly.
